HEALTH CARE PLAN
The City of Mansfield operates a self-funded health insurance plan for its employees and their dependents. The plan provides medical, prescription drug, vision, and dental (employee-only) coverage for covered persons.
Third Party Administrator
E. S. Beveridge and Associates, Inc., 111 Park Avenue East, Mansfield, Ohio is the plans third party administrator and is responsible for paying all health care claims on behalf of the plan. Diane Ward serves as the plans account manager. Diane can be reached at 419-525-3961.
Provider Networks
The plan participates in the Community Coordinated Healthcare (CCH) program. Call CCH at 1-800-247-8956 to:
- Make sure you have a referral for
- speciality physician services
- inpatient mental health and substance abuse services
- therapy
- Be pre-certified for:
- Inpatient and extended care facility admission
- Surgery
- MRIs/MRAs
- Chemotherapy
- Physical therapy (if more than 15 visits)
- Home health services
- DME rentals and any purchase over $500
- Hospice care
- Organ transplants
- Find out if a doctor, hospital, or other healthcare provider participates in the program, or with any other healthcare questions.
In addition to Community Coordinated Healthcare, the plan also belongs to the Direct Care America (DCA) network, a nationwide network of health care providers. DCA can be reached at 1-800-266-5896 or on the web at www.directcareamerica.com.
Caremark is the plans pharmacy benefit manager. For prescription drug claims or coverage questions, contact Caremark at 1-800-237-2767 or on the web at www.caremark.com.

"HEALTHY REWARDS" WELLNESS PROGRAM
The City of Mansfield Health Care Plan sponsors the "Healthy Rewards" Wellness Program. This program offers employees and their spouses the opportunity to receive bi-annual wellness screenings at no cost. These screenings can alert employees to potential health problems and assist them in maintaining or improving their health.
As an incentive, persons who pass these screenings receive reductions in their monthly insurance premium.

Changes in Personal Information
Employees must inform their supervisor, Human Resources and Payroll of changes in their personal information, especially information which can affect benefits, as soon as possible after the change occurs.
All employees must enroll their new dependants in the health care plan within 30 days of birth/adoption, etc. Enrollment is done by the employee in the Payroll Department, on the 7th floor of the Municipal Building.

WORKERS COMPENSATION
Employees who become ill or injured at work are to notify their supervisor as soon as possible, and no later than the end of the work shift during which the injury or illness occurs.
