HEALTH CARE PLAN
The City of Mansfield operates a self-funded health insurance plan for its employees and their dependents. The plan provides medical, prescription drug, vision, and dental (employee-only) coverage for covered persons.

Third Party Administrator
E. S. Beveridge and Associates, Inc., 111 Park Avenue East, Mansfield, Ohio is the plan’s third party administrator and is responsible for paying all health care claims on behalf of the plan. Diane Ward serves as the plan’s account manager. Diane can be reached at 419-525-3961.

Provider Networks
The plan participates in the Community Coordinated Healthcare (CCH) program. Call CCH at 1-800-247-8956 to:

Plan Administration:
The City of Mansfield Health Insurance Cost Containment Committee advises the Mayor on plan administration and annual funding decisions. The committee is currently made up of the following members:

"HEALTHY REWARDS" WELLNESS PROGRAM
The City of Mansfield Health Care Plan sponsors the "Healthy Rewards" Wellness Program. This program offers employees and their spouses the opportunity to receive bi-annual wellness screenings at no cost. These screenings can alert employees to potential health problems and assist them in maintaining or improving their health.

As an incentive, persons who pass these screenings receive reductions in their monthly insurance premium.

WORKERS COMPENSATION
Employees who become ill or injured at work are to notify their supervisor as soon as possible, and no later than the end of the work shift during which the injury or illness occurs.
